Meals
Please note that the following meals are included in the registration fee for Delegates, Affiliates, Advanced Trainees in Cardiology, Fellows in Training and Faculty:
Welcome Reception – Wednesday 06 June – please note this is for one hour only and includes canapés and drinks. Dinner is not included after the Welcome Reception and is at your own arrangement and cost.
4 morning tea breaks and 3 afternoon tea breaks (Wednesday - Saturday inclusive).
4 working lunches (Wednesday - Saturday inclusive).

Breakfast is not included as part of the meeting but coffee and muffins will be available for late risers.
Children
Children are welcome to accompany delegates to the meeting, however all meeting sessions, lunches and evening functions are “adults only” and children are not permitted.
CME/MOPS Points
For cardiologists and other physicians, RACP MyCPD Program Participants may claim credits for attending the 2018 Port Douglas Heart Meeting & Expo under “Category 4: Group Learning Activities” at 1 credit per hour (please note Category 4 is capped at 50 credits per year; a copy of the PD18 registration confirmation will suffice as documentation of attendance). For sonographers, ASAR/ASA/ASUM CPD Program Participants may claim credits under “Code 1A: Attendance at national/state/international meetings...” at 1 credit per hour of educational activity. A certificate of attendance or receipt of PD18 registration & copy of the program will suffice as evidence of attendance. Certificates of attendance will be emailed to all attendees on the last day of the meeting.
Amendment and Cancellation Fees – The Events Authority
No penalty will apply to amendments or cancellations prior to Friday 06 April 2018.
On or after Friday 06 April 2018:
Amendment fee: $50.00 per amendment per person
Cancellation fee: 25% of the total registration fee plus supplier cancellation costs
On or after Friday 27 April 2018:
Amendment fee: $100.00 per amendment per person
Cancellation fee: 50% of the total registration fee plus supplier cancellation costs
On or after Friday 04 May 2018:
Amendment fee: $150.00 per amendment per person
Cancellation fee: 100% of the total registration fee plus supplier cancellation costs
An amendment fee will be charged if there is a change to an existing registration confirmation ie date change of hotel booking, name change of delegate etc.
The above amendment and cancellation fees for The Events Authority are in addition to any fees which may be levied by the hotels in the event of an amendment or cancellation.
Please note the following cancellation fees for accommodation at the Sheraton Grand Mirage Resort, Port Douglas and QT Port Douglas:
Up to 61 days prior to arrival - Cancellation fee of one night’s accommodation
Between 60 days and 46 days prior to arrival - Cancellation fee of two night’s accommodation
Between 45 days and first date of reservation - Cancellation fee of 100% of reservation (no refund)
No-show on first date of reservation - Cancellation fee of 100% of reservation (no refund)
Depart the hotel earlier than confirmed reservation - no refund.
Credit Card Administration Fee
Registration and accommodation costs can be paid by credit card. A 1.3% plus GST totalling a 1.43% administration fee will be added to any Visa, MasterCard or American Express transaction.
Please note that the provider name “The Travel Authority” will appear on your credit card statement.
Liability – The Events Authority
The Events Authority is a division of The Travel Authority Pty Ltd. The Travel Authority Pty Ltd welcomes the opportunity to provide a travel service to their clients. However, persons who take advantage of this service do so with the knowledge that The Travel Authority Pty Ltd acts only as an intermediary, whose function is to make reservations with companies or firms offering facilities for accommodation and air travel associated with the travel industry. The Travel Authority Pty Ltd, whilst acting in good faith, does not accept any responsibility for default causing loss or injury to persons or property whether by negligence or otherwise on the part of any company, firm or any individual providing any facilities for any person taking advantage of the travel service. Further, The Travel Authority Pty Ltd is not liable for any loss suffered by any person or any additional expense caused or occasions by delays, change of aircraft services, alterations to any other travel services, illness, injuries, strikes, acts of terrorism, civil disturbances of whatsoever nature or force majeure interfering with, altering or adding to the cost of the service or accommodation requested or booked.
“Force Majeure” means (without limitation) an Act of God, fire, flood, tempest, blizzard, lightning, war, revolution, act of terrorism, riot or civil commotion, strikes, lock-outs or other industrial action, failure of supplies of power, fuel, transport, equipment, raw materials or other goods or services or grounding or delay of flights by airlines or by government authorities or operators of airports.

Travel Insurance
You must take out travel insurance. It is strongly recommended and a condition of your attendance to the Event operated by The Travel Authority Pty Ltd, trading as The Events Authority (ABN 66 107 889 707) (referred to as "TTA").
By confirming your attendance, you agree and acknowledge that:
1. You should have, or will at the time of travel have, a travel insurance policy suitable to cover any loss which may be suffered by you in travelling to and attending the Event (a "Travel Insurance Policy");
2. The Travel Insurance Policy you have, will provide cover against losses including but not limited to theft, damage, injury, illness, death or accident, associated treatment and travel costs and will cover you at all times whilst travelling or attending the Event;
3. You are personally liable to cover any and all medical and associated costs whilst travelling to and attending the Event unless this is covered by your Travel Insurance Policy;
4. TTA is not responsible for and has no obligation to arrange a travel insurance policy to cover you at any time in relation to the Event;
5. TTA is a booking agent and accepts no liability in connection with your related travel and attendance at the Event, for any:
a. loss, theft, damage, injury, illness, death or accident; or
b. direct or indirect financial loss, consequential loss, loss of enjoyment, pain and suffering, delay or irregularity; and
6. You indemnify and release TTA from any and all claims, suits, demands, liability, loss, damage, cost or liability for any loss suffered or incurred by you, in relation to or incidental to the Event.
Terms of Business
Please note that all bookings for travel made by you with The Travel Authority Pty Ltd (ABN 66 107 889 707) (TTA) are subject to our Terms of Business, unless you are a corporate customer who has entered into a separate travel management agreement with us (in which case the terms and conditions of that agreement will apply). By making a booking or requesting that we provide goods and/or services to you, you are taken to have accepted our Terms of Business.

Privacy
Information collected in respect of proposed participation in any aspect of the 2018 Port Douglas Heart Meeting & Expo will be used only for the purposes of planning and conduct of the Heart Meeting. If you wish to be excluded from a mailing list to be provided to Trade, please indicate in the registration section that privacy is required and you will be excluded.
